Dakota County Social Services-Children and Family, Transition support services for adolescents in foster care as they enter independent living.  Services can be extended to age 21 if the adolescent is attending high school or an equivalent educational program.  The program works with adolescents who are 16 years or older and are in permanent long-term placement settings.
